چکیده

Fifteen per cent. of all cases of Rh-isoimmunization end in stillbirth. Until 1963 the only available means of preventing this outcome in cases with a high risk of stillbirth was to deliver the foetus prematurely before intrauterine death had occurred. The value of this was limited, because many intrauterine deaths due to haemolytic disease of the newborn occur early in pregnancy, and because in the event of a live birth prematurity is a serious danger. The introduction of intrauterine transfusion (Liley, 1963) offered the possibility that intrauterine life could be prolonged to a stage where premature induction could be more successfully employed, but implicit for its success and justification is a reliable forecast of the possible stillbirth and of the probable time of intrauterine death. The present communication describes our selection of patients for intrauterine transfusion and our experience of the procedure as employed in the Newcastle hospitals during 1965 and 1966.

Rh alloimmunization and term delivery.

Nowadays management of severe Rh alloimmunization consists of serial determination of middle cerebral artery peak systolic velocity, amniocentesis, cordocentesis, and in many instances intrauterine transfusion. We present a case of severe Rh alloimmunization who, for the first time in Iran, was delivered at term after several intrauterine transfusions.

Introduction of a programme for intravascular transfusions at severe rhesus isoimmunization.

Thirty-seven fetuses with severe rhesus isoimmunization with a gestational age of less than 30 week underwent 92 intravascular transfusions. Of these, 77 were intracardiac, 13 umbilical vein and two umbilical artery transfusions. Procedure related complications occurred at eight (10%) intracardiac and at two (14%) umbilical cord transfusions. Reversal of hydrops was observed in 10 of 16 fetuses...

Consequences of fetomaternal haemorrhage after intrauterine transfusion.

Fetomaternal haemorrhage was studied after 68 consecutive fetal intravascular transfusions performed in 20 patients with Rh isoimmunisation. alpha Fetoprotein concentration was assayed in maternal blood taken before, and immediately after each transfusion and three and 24 hours later. An increase of 50% or more in the concentration in any of the samples after transfusion was considered to indic...
